While many women are familiar with saline or silicone gel breast implants, a new type of implant is now available—the IDEAL IMPLANT® Structured Breast Implant. For years, women have had to choose between the realistic look of silicone gel and the peace of mind that comes with saline. But now, an innovative implant option offered by Dr. Steven Ringler gives women the beauty they want, with the confidence they deserve.
The Downside of Silicone Gel
Silicone gel implants are known for their natural look and feel, but inherently come with the risk of “silent rupture”, no matter the size of breast implant. A “silent rupture” occurs when sticky silicone gel leaks into a woman’s body without her knowledge, and it can only be detected with an MRI. This is why the FDA recommends all women with silicone gel implants receive an MRI scan every two years for the life of their implants. When a silicone gel implant does develop a leak or tear, surgery is then required to take out the damaged implant and remove the silicone gel that gets trapped in the surrounding tissue.
When a Saline Implant Leaks
A “silent rupture” cannot occur with a saline implant. While saline implants are known to feel less like natural breasts than silicone gel implants, many women choose them for their security. If the implant develops a hole, the saline is harmlessly absorbed by a woman’s body. Plus, no MRI’s are required to detect rupture. Board-certified Dr. Ringler states:
“I’ve taken out hundreds of ruptured silicone gel implants during my career. It’s a big operation, removing the silicone gel. If I never see another silicone gel implant related problem, I’d be very happy. With the saline implant, if it leaks, it’s a very easy thing to change. There’s minimal downtime, and it’s minimal inconvenience to the patient. A saline implant is not subject to monitoring. They come in for an exam, they look at themselves in the mirror, and they can tell if their implant is leaking. I do think that the double-lumen of the IDEAL IMPLANT is nice because it’s never going to totally deflate. Patients don’t have to worry about that. It’s still going to maintain roughly 60% of its volume, even if the outer shell leaks. That seems to be reassuring to some patients.”
Similarities and Differences
Like a saline implant, the IDEAL IMPLANT requires no regular MRI scans and never leaves women worrying about “silent rupture.” But, as a “structured” implant, it provides all the best qualities of both implant options and it comes in a variety of breast implant sizes. The IDEAL IMPLANT allows women to achieve the look and feel of natural breasts, as with silicone gel, yet with the reassurance of saline as a filler.
IDEAL IMPLANT Structured Breast Implants are comprised of a series of shells nested together that hold two separate chambers filled with saline. This unique design makes the IDEAL IMPLANT move and behave differently than a saline implant. Saline implants can cause the skin to wrinkle and ripple. Saline implants can also act like a water balloon (the “water-hammer” effect). Furthermore, saline implants can collapse when upright due to the lack of any internal structure. The IDEAL IMPLANT addresses all these common concerns.

Dr. Ringler says:
“As soon as the IDEAL IMPLANT became commercially available, I began using the product. It’s a great advancement because it’s really the best design change that I’ve seen in implants throughout my entire career, especially being a big fan of saline implants … If someone comes in asking for a silicone gel, and I think their physical dimensions are a good fit for the IDEAL IMPLANT, then I generally tell them why I think the IDEAL IMPLANT is as good. I let them feel the implant. I show them the differences between the implants, even the same size IDEAL IMPLANT sitting next to a silicone gel implant has a different shape. I explain to them why I think the shape is better, and then I show them clinical photos of patients with standard implants and the IDEAL IMPLANT. The uniform response is, ‘I like the IDEAL IMPLANT results better. Those look better.’”

Determining Breast Implant Sizes

“I try and manage expectations by getting a really clear understanding about what the patient’s goal is and what their expectation is. Size is a big factor. We use computer imaging to help with that. We also use external sizers and clinical photographs to help with that. Patients will occasionally bring in photos that they think look good, that they’ve downloaded, and I’ll review those with them. I think the most important thing is that the patient has a pretty good understanding about what the size and shape of the breast is going to look like and if they need a lift or they don’t need a lift. It’s really important that they understand that. I think that if you’re really good at communicating those points with the patient, and have a long conversation about it, they’re going to be happy with the result.”
