What is Cosmetic Surgery with the IDEAL IMPLANT® Really Like?

Before I put the IDEAL IMPLANT into the patient, I first use a test implant which we call a sizer. A sizer is a temporary implant that we put in before we put the IDEAL IMPLANT in. We inflate the test implant up, that allows us to see the size. Is it correct? Is the pocket correct? Do we need to do any adjustments before we actually use the real IDEAL IMPLANT?”

The IDEAL IMPLANT® does not come pre-filled like silicone gel implants do. This allows the surgeon to adjust it during the procedure to accommodate differing breast implant sizes. How is the implant filled to the desired size? 

“The only person who touches the implant is the surgeon and this implant is kept inside its container until it actually goes into the patient. The IDEAL IMPLANT has two chambers that the saline (the saltwater solution) is put into. Once the fill tubes are in the IDEAL IMPLANT, the implant has to be folded in order to insert it. I will fold it so one tube is tucked inside one way and the other tube is tucked inside the other way. This makes it very easy for me to insert the IDEAL IMPLANT into the pocket. Once I have the proper amount of fluid, the fill tubes are removed and the fill valves are sealed leaving the implant with the desired size.”

“No woman has exactly perfectly matched breasts. Every woman’s breasts are asymmetrical. One of the great advantages of the IDEAL IMPLANT is that because it’s a structured implant I can add more volume to one breast to better match the opposite larger breast. Sometimes that amount could be equivalent to a half of a shot glass, or 15 cc.”

Can you explain the recovery process after breast augmentation cosmetic surgery?

“After surgery, the patients go to recovery for about an hour or so and then most patients are going to go home. I encourage them to get up and walk around that day and do a number of their normal activities. Most patients can return to brisk walking and to a desk job in two to three days after having their IDEAL IMPLANT breast augmentation and most patients can return back to the gym to full activities at three weeks.

If a patient comes in seeking a breast augmentation, how do you help guide them through the process of selecting implant type, size, etc?

“We have many women that come to our practice on a daily and weekly basis, seeking breast augmentation, breast lifts with augmentation or reaugmentation of their breasts. We always meet with the patient to obtain their desires, their concerns, and then I meet with them personally. Measurements are taken, the breasts are examined and a plan is made for the patient. As part of that, we provide the patient with all different implant possibilities to choose from. “I feel it’s important for every woman to make her own decision with regard to what type of implant they would like to use. We try to provide them with information to assist them in that process. “I first became aware of the IDEAL IMPLANT approximately 10 years ago. I have known Dr. Robert Hamas, the inventor of the implant, for many, many years, and he had shared with me the design of the new implant and what he hoped the implant would provide for patients. I was one of the early adopters of the implant as soon as it was available to the public, after going through its exhaustive FDA trials. I wanted to be one of the first surgeons to be able to offer the implant to patients. 

“So I have been using the implant for almost three years now. I find that it’s an implant that’s readily accepted by women, especially when they’re presented with all implants to choose from, and have a choice that they can make on their own. I also have husbands or boyfriends involved when they’re available, because I think it’s important that everybody have all the information possible to make their decision.”

How do you present the different breast implant types to your patients?

“I like my patients to be able to evaluate all the different types of implants, so I typically will show them a silicone gel breast implant, a more high profile implant, some of the newer models, and the cohesive implants. I also show them a standard saltwater implant (the type that’s been around for approximately 50 years, the single lumen unstructured implant) and I also show them a structured saline implant – the IDEAL IMPLANT. I present implants that are basically the same size so that it’s very clear what the differences are between the implants. Women have a chance to examine and feel the implants and feel the differences between all of them. I use the cloth that’s provided by the company to show the patient what these implants feel like underneath skin and subcutaneous tissue.

“I also let the boyfriends or the husbands feel the implants. And very frequently they have a very hard time telling the difference between a silicone gel breast implant and the IDEAL IMPLANT. I then also let them feel the difference between the unstructured saline and the IDEAL IMPLANT Structured Breast Implant in the same fashion and once again, it’s a very clear difference that they can discern right away. After presenting the patient with the options, it is very common for them to say, “Well, the IDEAL IMPLANT feels very similar to these other implants, so why would I want the silicone gel breast implant?”

How many patients would you say are selecting the IDEAL IMPLANT at your practice?

“A number of my patients who were originally considering silicone gel implants often end up choosing the IDEAL IMPLANT.  For women who want saline, when they’re presented with both the standard unstructured saline implant and the IDEAL IMPLANT they almost always choose the IDEAL IMPLANT. So I’ve found in my practice that I primarily use IDEAL IMPLANT Structured Saline Implants over the traditional saline implants. On occasion, if a patient is replacing a ruptured implant and wants to stay within the warranty provided by that company they may replace it with traditional saline, but otherwise I use mostly the IDEAL IMPLANT.

“I’ve also found that approximately 40% of patients who come in to speak about breast augmentation or revision augmentation surgery end up choosing the IDEAL IMPLANT. There’s still a place for silicone gel. There’s still a place, occasionally, for a standard saline implant, but a lot of my patients now are choosing the IDEAL IMPLANT. I see patients every week who come to this practice primarily because we offer the IDEAL IMPLANT and ask for it by name.”

What would you tell another surgeon who is considering incorporating the IDEAL IMPLANT into their practice?

“I’ve always felt that women need to be able to make their own choices. So I always present all the options to every woman who comes in. I may counsel them about what I think a wiser choice is, one implant to another based on their anatomy, based on their desires, based on the size they’re trying to achieve. And I think that that is something that’s driven my practice for many, many years. I truly believe in the IDEAL IMPLANT, the way that it’s been manufactured, the safety, the quality that’s been designed into the implant, the new shape of the implant which lets it sit on the chest wall, and be less palpable, and the overall feel of the implant.

“When I talk to other surgeons who may not be using the implant currently, I tell them that, at least in my patient base, this implant essentially sells itself. When patients have been presented with the information, presented with the implants to feel and to look at, most women choose IDEAL IMPLANT Structured Breast Implants when they want to go with a saline implant. I think that for any practice, the ability to be able to offer this implant is a real benefit.”

“I think that the majority of my patients that have chosen the IDEAL IMPLANT have been very pleased with their choice, very pleased overall. There’s less rippling, the implant feels softer. I think that the safety and the quality of the implant are paramount in their happiness with their results as well. There are a lot of patients who live in my area that are very health conscious. Many of my patients are old enough that they still remember the implant crisis of the 1990s and the fear that was engendered regarding silicone gel breast implants. And I think with some of the more recent news about textured silicone implants, I think that patients are looking for a safer alternative and I think that that’s what the IDEAL IMPLANT provides us with.”

What else would you like prospective patients to know about the IDEAL IMPLANT?

I think another benefit of the IDEAL IMPLANT is that they come with a tremendous warranty program. It’s a lifelong warranty program, and has been well thought out. 

“Another aspect of the implant that is beneficial for women and one reason that I think that they choose the implant, is that with a saline-based implant, you do not need to consider replacing the implant every 10 years, as is the recommendation from the FDA regarding silicone gel breast implants. I think this reduces the concern that women have that they’re going to have to spend time and more money in the future replacing their implants on a periodic basis. With a saline-based implant, you can just look in the mirror and know whether your implant is intact or not. With silicone gel, there’s something called silent rupture, where you can have a ruptured silicone gel breast implant and be unaware of it.

“Even the studies that are done to try and ascertain whether an implant is intact or not, such as an MRI, can be wrong. Recently, when a few of my patients had MRIs done to see whether an implant was intact, the MRI said that the implant was entirely intact, but when we went to surgery and operated, the implant was actually ruptured. And I had another patient, two, in fact, recently, that had an MRI saying that they had ruptured silicone gel breast implants and we operated to change their implants and found that both implants were entirely intact. 

“As far as breast implant revision goes, it is also a lot easier to replace a structured saline implant over a silicone gel implant. When somebody has a ruptured silicone gel breast implant, especially the older implants which were liquid silicone gel, it is a very difficult replacement surgery. They may need a total periprosthetic capsulectomy or removal of the implant and reconstruction, which is a much more difficult procedure than swapping out a deflated saline implant. Since it is filled with saline instead of silicone gel, replacing a saline implant requires just a slight revision and then a quick replacement of the implant.

“I think having a structured saline implant that feels more like silicone gel that women are comfortable with, and having an implant that is safer than silicone gel, is a real benefit to women. And I think that that’s why they’re choosing this implant over silicone gel. I think it’s due to health concerns and safety concerns. The longevity of the implant and the fact that they do not need to periodically change the implant as is recommended by the FDA for silicone gel, is a real benefit to patients as well. “I had two main concerns that led me to not want a silicone gel implant. Number one: I did not want to have an MRI every three years to check on it. I’m a mother of two, I work, and I don’t have time to worry or think about what’s in my body. Number two: the rupture cases that I’ve seen while working for a plastic surgeon and the amount of cleanup that he has to do for some of them is pretty gross.”

“I first learned about IDEAL IMPLANT through my work as a patient care coordinator, and working with Dr. Rossi. My Procedure

“I had the surgery done on April 2, so I’m just at about 12 weeks now. I feel better than ever. I always joke with Dr. Rossi, “I wish I had this done 10 years ago,” and he says, “Then you wouldn’t have had me”. And I love that because everything happens for a reason and timing is everything. I truly am so grateful for the outcome. My recovery was super easy. I did have to get a full lift, so I have the full anchor scar which can be concerning to some women, but it was necessary. Dr. Rossi and I agreed that it was the best option for what I wanted because I did want to go smaller. The recovery was great. It was actually really, really easy, and I would do it again.

“The main reason why I chose to have a breast lift is because I wanted to go smaller. After having two children, the skin stretching was just a little too much. I just wanted to go back to my previous size so I could feel like myself again. I think the biggest concern that most women have about a full breast lift is the scarring. But that’s why you seek out a highly reputable surgeon with impeccable suturing skills like Dr. Rossi. He does all the suturing himself, which says a lot about his style. I think that’s why I have the outcome that I do and why I am so okay with having these scars. And I know that with my fair skin, over time they’ll just fade to nothing.”

The Recovery

“To be really honest, my recovery has truly been a walk in the park, and I’m not just saying that. I mean, I’m sitting here three months out, and I don’t even wear a bra anymore – they are perfect. My breasts stay exactly where they need to be, and I don’t ever have to worry about them. So, it was maybe a couple of days of some discomfort, but it was nothing that a pain medication couldn’t fix. I went back to work five days later, and felt totally fine.”

My Advice 

“It’s important to know that you have a choice and you have options, and it’s not what we’re used to hearing all these years. Breast implant technology has come a long way, and now you can get the best of both worlds with the IDEAL IMPLANT. You can get the softness, you can get the perkiness, and still have that peace of mind knowing exactly what your implants are doing any day. I think women need to know that there is more than one option, and they can have a saline-filled structured implant without having to worry about rippling or that water balloon bag look that used to come with the old school saline implants.

“I think it’s really important to seek out a doctor that is doing this regularly and has a sincere passion for it. And that’s why I sought out Dr. Rossi to do my case. I also feel like it’s important that the breasts look natural, they feel soft, and that they don’t pose any health risks. There are no health concerns connected to the IDEAL IMPLANT, so that should be option number one for anyone who is worried about that. I know there are a lot of things you hear in social media about breast implant illness, or other concerns, but it gave me peace of mind knowing that the IDEAL IMPLANT has no connections to any adverse effect. It requires no monitoring, and has no new known association of complications, which I think is really important.”

Meet Dr. Rossi

“I’m originally from Chicago, Illinois. I completed my general surgery training in Chicago, followed by a highly intensive program there. My plastic surgery training was in Houston, Texas, in the biggest medical center in the world. That’s where I got my first real, extreme experience with cosmetic surgery. There’s a huge market for plastic surgery in Texas, and I really fell in love with the cosmetic surgical fields, breast surgery in particular.

“I’ve always had a knack for the cosmetic aspect of surgery, but I also really enjoy talking to my patients. I feel like a big part of my practice is helping patients connect the work that is done on their body with their mind, as far as how they feel they should look. That’s the biggest thing about my practice – making sure my patients feel that balance between the mind and the body.”

A Personal Approach

“I am very big on conversation with my patients and making a connection with them. I don’t have a one-size-fits-all approach to my patients. I spend an hour to an hour and a half with all my consults so we can get to know each other. I really get to know them on a personal level first, and that helps lead me to a lot of my answers.

“So if they are big into crossfit, if they carry their child around, if they don’t like to work out much, if they surf, if they hike, all these different things tell me a little bit more about their lifestyle. This helps me know what style of implant, what kind of approach, and what pocket to use. I use that information to then lead me to my measurements, which I’m very meticulous about. Then we discuss what it is that they want to accomplish.

“A big part of my mantra is the natural appearing breasts. Most women here don’t want to look overstuffed or augmented, they want to have a natural look. So I spend a lot of time analyzing my patient’s full body from their shoulders to their waist and hips, and explain to them what size, shape, and contour would be best to fit that whole package.”

My Introduction to IDEAL IMPLANT

“At baseline, I’m a bit of a science nerd. I really like to do my research, read journal articles, and see data out there. On Instagram I saw some posts from colleagues about IDEAL IMPLANT and when I learned what the implant was, it was fascinating to me. I thought it was a brilliant idea.

“So I did my own research on the IDEAL IMPLANT, and it really made me feel good that it was developed by a plastic surgeon. Because I think surgeons think very differently, more outside the box compared to the big corporations. I’m very patient centric, and I trust other plastic surgeons who are doing the same work I’m doing, and who also care about their patients. Whereas sometimes I think although companies want to make a good product, the bottom line is about making money. I got a sense of comfort seeing that IDEAL IMPLANT was made by a plastic surgeon.”

A Look at Silicone Gel Implant Rupture

“When I first came to San Diego, some of my biggest cases where these big revision cases. These women would come to me who had other consultations where they weren’t even told that they had capsular contracture. So they were a little shocked when I told them their implants were ruptured. Sure enough, almost every time I operated, they had an older model silicone gel implant where the gel wasn’t as cohesive. And not only did they rupture, but here’s the scary part: the older silicone gel implants’ shell actually sweat. Even though you can’t see a defined rupture within the implant, you can see that there’s this gooey gel-like sweat that’s outside the implant within a hard calcium capsule around it.

“That situation creates quite a problem for these patients. These types of patients are the ones you’ll see with implant related illness as a reaction to the silicone gel. The body walls it off appropriately, it just makes for very lopsided, unattractive looking breasts that are imbalanced. Many times it will progress to a grade four capsular contracture, which creates pain in the breasts. I have seen a lot of that. That is one thing that’s nice about IDEAL IMPLANT; that’s not something you have to worry about.”

Guiding My Patients Decisions 

“I’m seeing a lot of women who are mistrusting of silicone gel because of the news we keep hearing about breast implant related illness, capsular contracture, and now there’s ALCL. All these things are real concerns. With everything that is going on, the conversations I have with my patients have changed a lot over the past year. Women who want the IDEAL IMPLANT  tend to be women who like to feel very natural.They feel safer knowing that there is an inert substance within their body and that they don’t have to worry about getting MRIs because there’s no chance of a foreign substance such as silicone gel leaking into their body.

“With that said, I’m still using a lot of silicone gel in my practice because the silicone gel implant technology has changed drastically, in particular, in the last three to five years. But if a patient really is averse to having silicone gel in her body, then it’s a very natural decision for me. The only saline implant that I really would ever consider using moving forward is IDEAL IMPLANT. Once you can adjust to that and know what that looks like inside the body, then it’s no different than every other implant that you place. It just has a different sort of profile, that’s all.”
