Finding the Right Bra Size With a Cosmetic Surgery Expert

Dr. Sophie Bartsich is a board-certified plastic surgeon in New York, New York. The Root of the Problem

Dr. Bartsich explains the history of bras, dating back to the days way before cosmetic surgery when women wore corsets to support their breasts and entire upper bodies. Bras were introduced in the 1930’s and were made to fit a woman based on how much her breasts sagged. This system was only partially helpful. When the bra sizing system began to include breast volume as an important factor in cup size, the “A, B, C, D” letter system was introduced.

How Bras are Sized Today

If you want to figure out your cup size, first measure the circumference of your chest right below your breasts regardless of your breast implant sizes (if you have them). Then, move the tape measure up and measure the circumference of your breasts at their peak. Your final size is based off these two numbers: if their difference is one inch, the cup size is A, if it is two inches, it’s B, and so on. “This approach is simple and direct, and seemingly adequate, but ultimately totally inefficient,” Dr. Bartsich says.  

What We Should Be Measuring

According to Dr. Bartsich, the most important measurement that bra sizing does not take into account is the width of the base of the breast. She says, “As a plastic surgeon, this is the most critical assessment I make on any breast exam during a consultation. The base diameter of the breast sets the stage for any implant-based surgery by letting the surgeon know how much real estate she has to work with … Much as a foot fits into a shoe, width can make or break its success; cup sizing as we know it takes no account of this.” Because the options for breast implant sizes are so varied, patient and doctor both need these important measurements to achieve the desired results from cosmetic surgery.

Women with breasts that hang are hard-pressed to find a bra that fits. Current methods for bra sizing do not consider how much ptosis, or sagging, the breasts have. If a woman’s breasts are mostly situated lower on the abdomen rather than on the upper chest, a bra that is supposed to be the correct size based on her measurements will be far too small.

Public Enemy Number One

Gravity is, and always will be, an enemy to women trying to look their best. Its effect on our breasts is deflating and over time, our breasts will inevitably begin to sag. Dr. Bartsich says this creates even more of a bra sizing problem, especially for flatter breasts. “If you go back to the cup measurements, and you visualize the measuring process, it becomes clear that perky breasts will stick out more than flatter ones. And they could easily measure out to a larger cup size even if the breasts they represent are in fact half the size of the sagging ones.”

Do Lingerie Stores Have the Answer?

Some women prefer going to to their favorite lingerie store to have their busts personally measured to find the perfect fit. This can be helpful, because the sales associates know their specific brand and how it will feel on you. But not every brand will fit each woman the way she wants, especially if she has different breast implant sizes. Dr. Bartsich says, “As long as we have the current system, finding the right size bra will continue to be similar to finding the right size pair of jeans. The sizes will serve as a guideline, but it is only once you identify the perfect brand for you that things will really fit into place.”

According to Dr. Bartsich, the lingerie assistants provide only a “band-aid” to the real problem or bra sizing because women’s breasts will never fit into a small, medium, or large size chart in real life.

There is Hope in the Future

Because she dedicates her work in cosmetic surgery to helping women feel comfortable and confident in their own skin, Dr. Bartsich wants to be part of the solution. She says, “It is my plan to devise a realistic bra sizing system in the near future, one that can be standardized and represents relevant measurements for each woman.” She wants women with natural breasts and those with different breast implant sizes to have a go-to bra size. She does not expect the task itself to be the most challenging part; it’s getting the world’s bra manufacturers to accept the new system that could prove difficult. It is possible, but the switch could take some time.

Silicone Gel Breast Implants

Dr. Galumbeck explains why women liked the silicone gel breast implant when it was first introduced, “In 2007, the silicone gel implants came out and the advantage is that because it’s more cohesive, it has a more natural look, more natural feel.” Silicone gel is a thick, sticky substance that provides more stability to a breast implant than saline. The result is an implant that does not ripple or wrinkle and looks natural. Since the FDA approved silicone gel breast implants they have grown in popularity. However, these implants have a downside which Dr. Galumbeck explains, “The disadvantage is that if it leaks, the silicone gel stays in the breast tissue and so you can’t tell that its leaked with a ‘silent rupture.’ The FDA actually recommends that women get MRIs every three years to look for leakage.” Women do not like the hassle and expense of repeated MRIs, which cost on average between $444-$1468. In addition, the anxiety caused by not knowing the status of their breast implants can be a daily stress for some women.

Breast Augmentation Complications: How Implant Types Compare

Breast augmentation is not without potential complications. Additional surgeries may be required to address complications such as rupture, capsular contracture, or cyst removal. According to the FDA, rupture and capsular contracture are two of the top reasons for breast implant revision surgery. The breast implant you choose can give you a statistically higher or lower risk of needing a revision. Knowing statistics on rupture rates and capsular contracture rates can help you plan financially and prepare mentally for the possibility of additional breast surgery.

Unfortunately, many women are not aware of the high rates of complications with silicone gel breast implants. For primary augmentations, silicone gel breast implants have a capsular contracture rate of 10.9-16.2%, and a rupture rate of 7.2-13.6% in 7-8 year trials. Ruptured silicone gel breast implants can also lead to other complications. The FDA lists these possible complications as, “a decrease in breast size, change in breast implant shape, hard lumps over the implant or chest area, an uneven appearance of the breasts, pain or tenderness, tingling, swelling, numbness, burning, or changes in sensation.” It is important to get the recommended MRIs every three years to check for a silent rupture before it turns into a larger issue.

The lower rates of these major complications mean you are at a lower risk of lifetime surgeries. Breast Augmentation Surgery and Recovery

“The procedure is very safe. It has been done for many, many years. The recovery is quick. During the operation that we perform here at our fully accredited facility, I use an endoscope, which is a specialized TV camera, to look inside. So I keep the incisions smaller, and don’t have to use drains afterwards. We have a hyperbaric oxygen unit that we have patients get in after surgery to help speed the recovery process.”

The surgical process typically includes the following steps:

    • Preoperative Markings: These markings ensure your breast implant is placed properly.
    • IV Anesthesia: A physician anesthesiologist gives intravenous anesthesia.
    • Incision: An incision is used to gain access to breast tissue. There are four choices for incisions—axillary, periareolar, inframammary, and transumbilical.
    • Creation of the Breast Pocket: A pocket is created to place the implant into. Dr. Galumbeck uses an endoscope to allow for precise dissection.
    • Inserting the Breast Implant: The breast implant is inserted into the pocket.
      • Final Check: Surgeon checks to make sure breasts are symmetrical.
    • Incision Closure: The final step, closing the incision with methods to minimize scarring.

The whole procedure is finished in about an hour, after which you will be taken to a recovery room. After some observation patients are usually sent home the same day. Dr. Galumbeck tells us, “most women are back to work and social activities in a day or two after breast augmentation.”

Tell us a little bit about you and your practice.

I’ve been in practice for about 25 years, and in private practice in Manhattan for 20 years. I perform a wide range of plastic surgery procedures from face to breast to body on both men and women. I’ve developed quite a following in terms of my breast enhancement practice. Which includes primary augmentation as well as secondary breast work.”

Why do you think women choose your practice over other options?

“I look at a breast augmentation consultation as an educational process, with the necessity being that the patient learn about her options. Most people who come in for a consultation aren’t truly informed of their options and the trade-offs involved. I take into consideration a woman’s concerns and anatomy, and then I educate her in terms of what her options are. I try to honestly portray the pluses and minuses with each option so that she can make the best decision for her.”

What is the process like for a woman coming in for her first breast augmentation consultation?

“When a woman is having a breast augmentation for the first time, it’s very important for her to understand what her choices are in terms of implants. I was training in plastic surgery in 1990 when the FDA said not to use silicone gel implants for anything other than breast reconstruction. That remained true until 2006. Because of that, I learned very quickly in the 90’s how to use saline implants. Soon after I finished training, endoscopic plastic surgery hit the scene. I was one of the developers of endoscopic breast augmentation, which means putting implants in through the underarm. The saline implant is a very attractive implant because it can be rolled into a small cigar shape and put in through a very small scar. It is a wonderful implant when hiding behind the right breast. The issue with saline implants is that in some patients there were issues with rippling which could be visible. They also didn’t feel that natural. The other problem with it was a relatively high rupture rate.

“It is for these reasons that when the IDEAL IMPLANT came on the market I was very attracted to it from the beginning. This implant gives you the best of both worlds with its structure and design. Many women are concerned about implant safety. I respect the FDA greatly, but it tends to change its mind. We don’t always know the truth because the truth evolves in terms of our perception and over time. So many women look at silicone gel in a similar light and worry that the FDA says silicone gel is fine and harmless now, but who knows in 10 years if it will be? A woman that isn’t so comfortable with silicone gel now has a really, really good option that gives her the peace of mind of no silicone gel in her body and yet having an implant with an exceptionally low rupture rate, a very, very good feel, a very natural appearance, and no worrying about ripples. The IDEAL IMPLANT really presents a solution that the more I educate my patients on, the more I see that it’s a great solution for many, many women.”

How did you hear about the IDEAL IMPLANT ?

I learned about a couple of my peers who were already involved, and it was a very natural step for me to get involved as well.”

You need to understand that there is no all-purpose algorithm for treatment. Treatments need to be customized based on the concerns and values of the patients. I find that too many doctors these days do everything on a reflex action. That’s overly simplistic and it’s simply wrong. Patients need a variety of options.

“I think our practice stands out because we very much individualize our treatments. I’m very proud to be able to offer minimally invasive treatments that hide scars. I’m proud that I do virtually all my liposuction, and the vast majority of my facelifts, under IV sedation. I tell you to take a deep breath, roll over, and I beat the chances of airway problems or blood clot. I enjoy the act of surgery as well as the relationships with the patients that occur from the whole process. So I guess I’m a pretty happy guy.”

What is a “Silent Rupture?”

When a saline implant ruptures, the implant deflates and the saline (a sterile saltwater solution) is harmlessly absorbed by a woman’s body. This is not the case with a silicone gel implant.
