In this recent interview with Dr. Thomas Fiala, we got to know more about him and his renowned practice. He is part of a very exclusive group of surgeons who now offer both high resolution ultrasound technology (HRUS) for silent rupture detection and the latest in implant technology, the new IDEAL IMPLANT® Structured Breast Implant. Discover why the technology and design of the IDEAL IMPLANT are cutting edge for the Orlando breast implant world and how you can get access.
What does your protocol entail when you find a silent rupture during HRUS?
“We start with a conversation with the patient about if they want to keep their implants or remove them. If they want to keep them, then we talk about the surgery and what’s involved. During the surgery we will remove the old implant, clean up the gel, and do the capsulectomy. Then we’ll talk about what they want to replace their implants with and all the latest things that are available. I always bring up the IDEAL IMPLANT® Structured Breast Implant because I think it’s a really great alternative.”
How do you help your patients make decisions about their implants?
“I’ll talk with patients for as long as is needed to really understand what their goals are. I’ll measure them and use anatomic principles of matching the width of the implant to the width of the patient’s breast frame and breast shape. Once we know the best size, I’ll show the patient her choices. Choices include implant profiles, saline versus silicone, the cohesives, the IDEAL IMPLANT, etc. I go over the pluses and minuses of all the implants and we talk about what’s important to them and come up with a recommendation.”
What is it about the IDEAL IMPLANT that makes women want to choose it?
“It’s usually the patients that are thinking about a saline implant that, when I describe the IDEAL IMPLANT and how it feels and performs, they are very interested in it. The biggest group of patients that want the IDEAL IMPLANT are the young patients, aged 20 or 21, because they want an augmentation but they don’t meet that FDA 22 and up rule for silicone. With IDEAL IMPLANT, I have a product that I can offer them that I think gives a nice result, and I don’t have to worry about the ripple and wrinkle problem that you get with unstructured saline implants.”
How long have you been offering IDEAL IMPLANT and what’s your experience been with it?
“It’s been about two years that I’ve been offering it and the surgical part is fine. It’s no problem to deal with the two lumens, and the two tubes, and my scrub techs are very familiar with it. The packaging is clear, so you really know how much to put in each lumen and it’s very straightforward. I still use one of the funnels to place the implant and no problems with that. Post op-wise it’s routine. No different than any other implant as you go through the recovery process.”
What has been the patient satisfaction with the IDEAL IMPLANT?
“Overall, the women have been happy. They like the shape IDEAL gives and they feel good about the advantages of the saline implant that we’ve talked about at the consult. The patients that I’ve placed so far are doing just fine.”
What would you tell another surgeon about the benefits of the IDEAL IMPLANT?
“I would tell them that it’s a premier product. The data on it is solid and very convincing. There’s good reasons to use it from a scientific point of view and patients have been happy.”
Why do you think patients choose you over other practices?
“We’ve been here for 20 years and working hard to get each patient a great experience. For every patient I see, I really give them my wholehearted best effort every time we’re together. In terms of the surgery I’ve had a long history of bringing innovations into the practice at an early stage, before they’re widely adopted. Patients know that when they come here they’re going to get a good service, really good surgery, and the latest and greatest stuff that’s proven by science to work. I’m proud to say that we’ve been voted the number one surgeon in plastic surgery practice in the greater Orlando area five times in the past six years by the readers of our local Orlando magazine. What we do is partially technical and partially artistic so you have a little bit of both.”
