A Discussion With Dr. Kurley of Advanced Aesthetics Surgery and Laser Center

We spoke with Dr. James Kurley from Advanced Aesthetics Surgery and Laser Center about breast implants. Dr. James Kurley is a plastic surgeon in Champaign, Illinois, who has more than 20 years of experience in cosmetic surgery. What procedures does your office focus on?

“The number one thing that I do in my office is breast augmentation, followed very closely by liposuction and tummy tuck. Facial cosmetic surgery is only a small part of my practice. I enjoy it, but my focus is on breast surgery and body contouring.”

What is Rapid Recovery Breast Augmentation?

“I specialize in rapid recovery breast augmentation, which allows women to get back to work much more quickly after surgery. This is because of the methods we employ in the recovery process. Many women are able to drive themselves to their appointment the next day, lots of women are hitting tennis balls two, three days after surgery. I’ve even had four personal trainers teach kickboxing classes in the first week after surgery. So this procedure does not have to keep you down for weeks on end, and women can get back to work or get back to driving kids around and enjoying their day-to-day routine.”

How do you help women pick the right breast implant sizes for their body?

“I always give my patients a range and if they choose a volume that’s outside of that range, we have to talk a little bit more about that. I don’t want to exceed the base dimensions of the breast and then create problems that are extremely difficult to correct. Me and my staff will evaluate the breast, get some measurements of the height and the width of the breast in relation to the entire chest. And then we look at the charts and go by the charts in terms of what volume may fit their chest. And having high profile implants has made it somewhat easier to give a little bit better volume adjustment than we did when we only had moderate low profile implants.”

Can you tell us more about the 3D imaging system you use in your office?

“Our system allows us the ability to scan the patient’s torso. The scanning process is relayed to a computer system that then generates a 3D image and it allows us the ability to put in whichever implant we’re thinking about to give the person a better idea of what they can hope to accomplish. We can then take that image and, using Apple TV, relay the image to a false screen TV that we have oriented vertically. The individual can then stand in front of this like they’re standing in front of a mirror, look at their full dimensional torso and turn from side to side and see what they’re going to look like with the image of the implants superimposed over them.”

How do you help guide your patients to make a good implant choice?

“Well, in addition to having the 3D imaging, we give our patients quite a bit of time to go into our try on room where they can use the trial sizes of the range that we’ve suggested and see if it’s giving them the kind of look they’re trying to accomplish. Because a 32C is not the same as a 36C, is not the same as a 34C.”

There are many different breast implants that women can choose from, including silicone gel implants, traditional saline implants, and structured saline implants. What is the best option?

“Well I think that it really depends upon a number of factors. The amount of soft tissue coverage a woman has is the number one most important thing in terms of whether you’re going to get good results or not. If a woman has virtually no soft tissue coverage, she will need a very good saline implant in order to hide rippling and wrinkling that might occur within the shell of the implant.  If she comes in and has a very good soft tissue coverage, then I’m comfortable recommending either traditional saline, structured saline, or silicone gel. If they have virtually no soft tissue coverage, then we need to talk more aggressively about using gel and gel of a greater consistency.”

Many women are concerned about silent rupture. What can you tell us about that?

“A silicone gel implant can rupture and the individual can be completely unaware that it has happened. Some women just don’t want to deal with that uncertainty in their lives, and they’ll choose breast implants filled with saline over silicone gel for that reason alone. With silicone gel, if the implant does rupture, then it’s much more difficult to deal with, because you have to get the sticky gel substance out, and the associated scar capsule that has formed around the implant. Whereas if a saline implant leaks, all you have to do is slip out the old implant and put in the new implant, so the revisional surgery is much less complicated. I want my patients to be comfortable with whichever implant they choose.”

How long have you offered IDEAL IMPLANT® Structured Breast Implants and about how many of your patients have chosen this option?

“I had a patient who called mid-November and said ‘I want the IDEAL IMPLANT.’ And I said ‘Well, tell her I’ll look into it. I’m willing to explore it, and if she’s comfortable with it, we’ll give it a try.’ It was because someone called us specifically asking for IDEAL IMPLANT® Structured Breast Implants that we got involved in the first place. So I thank her for that. This means we have had access to the implants since December of last year, so we have about seven months experience. What I find interesting is that last month, out of the procedures I performed, about 50% of women chose structured saline IDEAL IMPLANT® Structured Breast Implants and 50% picked silicone gel-filled implants.”

Why do you like the IDEAL IMPLANT?

“I’ll be honest, these days if I’m going to put in a saline implant, it’s going to be the IDEAL IMPLANT, because it comes very close in approximation to the feel of silicone gel-filled implants. We were at an event just last week where we gave women the opportunity to feel the different implants. And the women were surprised that a structured saline implant could feel as close as it did to a silicone gel-filled implant. Plus, this implant doesn’t ripple and wrinkle like traditional saline implants do. So, I’m really pleased with the results. And more importantly, the patients who’ve had the implant placed are pleased with the results. My practice has always been about giving individuals choices and options. This implant takes the issue of silent rupture off the table, and still provides women with a very nice, relatively wrinkle free, natural feeling outcome.
